H.O.M.E. Hospital to Outpatient Made Easy® is a fully integrated patient education, pharmacy, and medication access service designed to support patients as they transition through the interfaces along the continuum of care. The program is designed to streamline access to post-discharge medications, deliver supplemental patient education, access to “live” pharmacist or nurse support, and provide reimbursement support and financial assistance for outpatient medications if needed.
